What do I need to know about scheduling an appointment?
We ask that you please schedule appointments in advance whenever possible, most especially for routine check ups and follow up visits. Obviously, if you are ill and need to see one of our physicians, please call the office. We offer same day appointments during office hours and in our after hours acute care clinic daily. We also request that you arrive at least 10 minutes prior to your appointment time in order to complete necessary paperwork for your insurance company and your medical record. If you are unable to keep a scheduled appointment, please call us to cancel or reschedule at least 24 hours in advance so we can make your appointment time available to someone else that needs medical attention. We reserve the right to charge a $35.00 fee if you do not show up for a scheduled appointment and do not call to cancel.
How do I request prescription medication refills?
For quality of care purposes, please contact your pharmacy 48 hours in advance to request prescription refills. The pharmacy will fax a copy of the prescription refill request to our office. Phone calls to our office for routine prescription refills are not accepted. We will require a minimum of 48 hours to review your prescription refill request. Please do not request refills or new prescriptions for acute illnesses, as it is our belief that quality medical care dictates evaluation and management of acute illnesses in the office. Please schedule an appointment either during office hours or in our after-hours acute care clinic.
How can I find out about my test results?
Lab test results will generally be given over the phone via our phone messaging system. Occasionally, we will require a follow up visit to review test results. If you are diabetic, have any cardiac or respiratory condition, are on cholesterol medication, or have any other chronic condition, your physician may want to see you to discuss test results and your treatment plan.
How can I get my doctor to complete specific forms?
Please check with the receptionist regarding forms that need completion by the doctor. Life insurance forms, disability paperwork, etc. may be subject to an administrative fee of $25.00. Forms typically will require 48-72 hours for doctor completion, as direct patient care remains his first priority.
